Tobacco Control Program Printed Materials Order Form
Physical orders have ended as of 8/7/2026 and will be unavailable until early 2027. All options below will be emailed as PDF copies for your agency to use for promotion. The department asks that no modifications are made to the documents.

Materials are categorized by type (i.e. brochures, toolkits, posters, etc.). Some materials may be limited in quantity, we will do our best to fulfill your order. Our Program sends requested orders out bi-weekly, please let us know if this is for an upcoming conference or special event.

Please provide responses only to the materials you would like to receive. Do not provide a response (0 or N/A) for unwanted materials.
